The banksman whoo has to repeat the signals from the bottom into the enegine house mistook a sound caused by an end bar of a tram being dropped into the tram for the signal from the bottom and accordingly signalled to go on. Deceased was taken some distance up the pit by the cage and crushed against the side of the shaft and then fell to the bottom.
