- Deceased was taking a set of six full tubs from out of a slant road on to the main endless haulage rope with a chain attached to it, when a set of six full tubs about 70 yards in front of him broke loose from the rope, ran back along a gradient 1 in 13, and caught him as he was escaping into a refuge hole. There were smallman clips at the font and back ends of the set; the front end one lost the grip of the rope and the back end one broke when the set fell back on to it. An automatic switch threw the tubs off the rails, and they travelled 50 feet before stopping. The front clip, which was put on the, rope by the deceased, was probably not tightened sufficiently at the start.
