- Deceased was found under the wheel of an empty truck which had been delivered in the belt-siding by means of a flying shunt by the locomotive. The truck, unknown to the shunter, had not run far enough to clear a crossing owing to the wheels being obstructed by small coal lying on the rails. The deceased appeared to have gone voluntarily to clear the small coal from the front of the wheels, when other waggons, which were being moved on an adjoining line, collided with the standing waggon, which moved forward and trapped him. He died on the same day.
