- The colliery locomotive had pushed forward a crane truck containing pump and four chaldron wagons of coke. The driver stopped his engine at a tank and deceased who was not usually employed as brakesman appears to have been uncoupling the truck containing the pump and the coke wagon next it when he was crushed. To uncouple a pin required to be taken out of a shackle and a coupling pole could not be used. The buffers on the truck and wagon did not correspond.
