A shot-hole, 3 feet 6 inches deep had been drilled 3 feet above the floor in a seam of coal 4 feet 6 inches thick. When the deputy was making the usual examination of the place the deceased fetched from the wayhead a canister containing 4.5lbs of compressed powder. He then struck his candle on the coal face near to the shot-hole and opened the canister which lay on the floor beneath it. The candle dropped into the canister and ignited the contents. Deceased received terrible burns about the face, head, and upper part of the body, to which he succumbed on May 4th. The deputy, who was only a few feet distant received severe burns but was able to leave Hospital and give evidence at the inquest. As the seam worked at the place of accident was only 4 feet 6 inches in thickness it is obvious that the candle could not have been at least 4 feet from the shot hole or explosive, as required by the Explosive in Coal Mines Order of October 15th. 1912.
