Deceased was employed to open and close a ventilation door on a jig-brow. He was about to open the door to allow a set to be jigged, when a runaway full tub struck the door and threw it against him, inflicting injuries from which he died on the following day. The full tub ran away owing to the failure of the man in charge at the top of the jig to use the Warwick prop provided to prevent such an occurrence.
