Deceased, who was an examiner on the night shift, combined with this duty that of hitcher at a mouthing in the shaft. He had sent all his men up and was about to ride himself, for which he had to give the signals himself. He appeared to have made a wrong or indistinct signal to the engineman, and the cage was moved away as ho was entering it. He was caught and dragged up, and crushed between it and the side of the shaft.
