He was in charge of a small auxiliary hauling engine drawing tubs from the coal face. The starting valve was so placed that if there was slack tail rope he had to reach over the rope to get at the valve. He was doing this and started his engine too rapidly, and the tail rope, when it drew taut suddenly, struck him under the chin and broke his neck.
