The full set coming into the shaft landing took the empty road. The deceased was behind the empty set, which was made up ready to go out, and was between it and other tubs. The full set bumped the empty set back and the boy was crushed. The switch was self-acting. It should always have been in the right or the set to take the proper road. When examined by the wagonwayman only ten minutes before the accident and also afterwards it was found in good working order. Careful inquiries failed to throw any light on the cause of this accident.
