- Deceased was engaged tailing tubs up a brow by means of a tail rope lashed to the main endless haulage rope. He had just unlashed the tail rope from the main rope, and was returning down the brow, when two full tubs became detached from a lashing chain about 50 yards up the brow and ran amain. Deceased was run over by these runaway tubs and so injured that he died before reaching the surface. The lashing chains and couplings were in good order, and there was no jerking or reversal of the rope to cause the tubs to be detached from the rope.
