Inspection made and inquest attended. Deceased was fatally injured by being struck by the haulage rope near the bottom of the shaft owing to three full tubs falling out of the ascending cage striking the haulage rope. The tumbler of the cage was said to have been slightly bent during the previous night and two witnesses at the surface stated they noticed the tumbler up when the cage reached the surface although the assistant hooker-on stated he had put it down.
