- A broken tumbler shaft forming part of a Lubrig washer was being replaced by a new one, which was lying on a wooden floor. The broken shaft was slung in an inclined position from blocks suspended from three legs. The latter were not secured to the floor, and as the lower end of the shaft was clearing an aperture between two girders 4 feet apart, it swung round and knocked against one of the legs, causing all three to collapse. Hordon was struck by one of the legs and fell against the new shaft on the floor, and while in this position the broken shaft crushed him and injured him internally.
