The cage in which deceased and another man were being brought up the shaft was overwound and was held by the detaching hook ring, but the velocity was so great that the deceased was thrown against the roof of the cage and killed, and the other man was injured. The winding engine was found to be in proper working order and the overwind was due to a mistake on the part of the engineman.
