- Head smashed by tram against the centre post of the headstocks. Apparently after putting an empty tram on the cage he let his foot remain on the lever of the catches for preventing trams running forward to the shaft until required to put on the cage and consequently as the rails are on a gradient for the purpose of the trams gravitating to the cage this one did so. The descending cage catching the front end of the tram caused the other end to fly up and crush his head as stated.
