- Deceased was being raised from the bottom of a shaft, 170 fathoms deep and 15 feet diameter, on a flat cradle, filling about one-third of the shaft area. His mate stood at one end and he at the other to keep the cradle from catching the shaft wall. This could be done with one hand, but his mate, feeling the cradle turning, looked round and saw deceased had both hands against the shaft side, and in consequence was pushing the cradle into the pit. He fell through the space into the shaft bottom 30 feet: below, and was instantly killed. Safety belts are provided, but were not used.
