Deceased was walking down a jig in the Cockshead seam. He and others were, with drawing from the face on account of a weight. He was just opposite a manhole 8 yards from the top of the jig, when a piece of roof 9 feet long, 12 feet wide, and 1 foot thick fell from a slip running across the jig, and buried him. Ordinarily, the roof in this part of the seam is strong and will stand over the packs without timber. In this instance, however, a ripping shot had blown into the roof somewhat on one side, and caused a weakness which should have been detected and provided against.
