Deceased was engaged at a siding between two inclines or brake hills, his duty being to receive the full tubs at the bottom of the first hill from the workings, and to brake these down the second hill. The top incline was run with one tub and the second with two tubs. He had received one full tub and lowered this on to the blocks, and was apparently pushing in the second empty to attach to the rope, when a runaway full tub crashed into the tub he was pushing so severely injuring him that he died on the way to the hospital. The runaway was apparently caused by the braker at top incline neglecting to close the stop block after sending down the last full tub.
