- Deceased had pushed an empty tub out of the cage, taken it 6 yards from the shaft to some flat sheets, and there twisted it on to another road. He then took down a full tub to the flat sheets and apparently forgetting which cage was down, he neglected to twist the tub and pulled it into the open side of the shaft and fell into the sump with the tub on top of him. He was dead when got out, having apparently been suffocated by the coal from the tub. The accident could not have happened had the sump been kept securely fenced.
