When working on a ledge he stepped backwards and fell to the bottom of the quarry a distance of 150-200 feet. He was provided with a rope which he had not attached to his body. I visited the quarry three or four weeks before, and finding that the men were not provided with ropes when working in such places caused the ropes to be supplied which was done without delay.
