Inspection made and inquest attended. Deceased and the fireman were examining the shaft and had stopped the cage about 120 yards from the bottom order to fasten a staple in a pipe when something fell down the pit striking deceased on the head causing him to loose hold of the chain on top of cage he was hold to and he fell down the pit and was killed.
