When shunting an empty timber train through the cage at the pit bottom, the hook became fast on the cage door. Deceased was attempting to release the hook when the cage went away unexpectedly, and he was crushed between the cage and the top landing at the pit bottom. The engineman received a signal on the pit bottom bell in the engine house. The signal was also heard at the pit bank, and was probably accidentally given from the pit bottom.
