- Two men were holing at the coal face which was properly spragged, and deceased was filling the coal into a tub at a gate when without any warning some, stone from the roof fell between the face and the timber. Ainsworth was instantly killed and one of the others received severe injuries to his back. The fall disclosed a slip about 3 feet from the face which could not have been detected previously.
