- Deceased was trailing two 6-feet rails down a haulage brow, when a stone about 4 feet square and 7 inches thick dropped from the roof and crushed him. He was found doubled up in a crouching position and had probably been suffocated. The haulage road was also the return airway, and the stone was damp and had fallen away at a slip running diagonally towards a refuge hole. The road had been in use about 15 months, and although no timber was actually under the stone, there were two supports close to it at the side.
