Whilst the deceased was entering the cage on the heapstead, at the commencement of the night shift, the engineman lowered the cage and fatally crushed him. The accident was apparently due to a misunderstanding regarding the signal given and to the keps not having been used on that occasion. The winding engineman stated he received two rings on the signal to lower the cage, but the banksman denied having signalled to lower the cage, when the accident occurred.
