Died 1st June. Deceased was head rolleywayman and foolishly put his hand and arm inside a friction clutch in connection with the haulage machinery. While in this position the rope was restarted and the deceased's arm was torn off. The accident occurred about 12 yards from the code of signals on which it was distinctly stated that certain signals were to be given to the engineman before any oiling or alteration of the machinery was attempted. Deceased had omitted to carry out these instructions.
