Details of Event
A locomotive engine was taking loaded side tip waggons down the line, dipping 1 in 30. Deceased was on the brake of the last waggon and was at the left and outside of the waggon he then went round to the first waggons and dropped the brake which was on the right side. Instead of going to the last waggon he put his foot on the lever and the wheels of ran over and fatally injured him.
