Inspection made and inquest attended. The deceased after pushing two empty tubs into the cage was said to have shouted ‘Right’ and the head breaksman pulled back the lever for the catches of which the cage rested and the engineman started to lower when the deceased got crushed. He appeared to have missed putting the catch right to prevent the tubs coming out when he got caught by the descending cage before he was able to manage it.
