He appears to have left his work on a dark morning without his hand lamp and by some means to have fallen into a manhole of a culvert 18 ft. 6 ins. deep and been drowned or suffocated. The covering was of 1-in. boards which, owing to the steam passing under them were dazed and it is probable he had stepped on them, and they collapsed. An iron grate will be put on the manhole in future.
