Fall of coal from face, 6ft. long, 3ft. deep) 20ins. across at top, 24ins. at bottom, two tons in weight came from a smooth slip. This formed upper part of seam. The lower part had been blown out in the fast by three shots fired at 9am. Deceased then cut one side, tried to fall the coal, failed, went under to further hole. Inclination 1 in 2.5. No sprags used or provided.
