Deceased had almost completed clearing away a fall from between the pack and the coal face, and was sitting at the extreme end of the fall, when a piece of stone 3 feet 6 inches long, 1 foot 6 inches wide, and 1 foot thick, fell from the roof. One end fell to the floor, and the other end struck deceased on the forehead. He died the next day from fracture of the spine at the 4th vertebrae.
