Inspection made and inquest attended. The wrong signal was given to the winder and he suddenly raised the tub the deceased and three other persons were ascending from the sump the tub being attached under the cage by a chain and as it was raised without steadying it caught a bearer and the chain gave way and the men and tub fell back to the bottom of sump the deceased being fatally injured.
