Deceased was passing the foot of a self-acting incline, when his was struck by a runaway tub and injured, which resulted in his death on 18th Nov. 1907. The cause of the runaway was due to the wheel round which the rope passed slipping on the wheel tree, and allowing this whole apparatus to run tree. This wheel was secured by a chain wrapped round this wheel tree, and the ends of this chain were nailed to this tree this nails came out and chain became loose.
