- A fault slip had been stripped in front of the gate road, and part way along the left bank. Owing to a wet and tender roof a small pillar of coal was left to support it. The ripping lip was taken down to the fault slip, and the right side pack built. Deceased was engaged arranging stone packers opposite the coal pillar, when the roof above collapsed. He was dead when taken out. Ripping past the stone gave it a loose end which pulled over a bar and prop.
