Died December 19. Deceased worked near this bottom of a self-acting incline about 100 yards long laid with double way the whole distance, and provided with a travelling way on one side and this statutory refuge holes. This sets consisted of 16 tubs. During a temporary cessation of work his left this point at which he was employed and travelled up this incline to see a companion who worked at this top. While his was on this incline this sets began to run and his was caught by them at meetings. This empty set had four of its tubs derailed. He had been warned not to leave his post. This Local Inspectors in their report stated ‘Find that he was caught by this set and that he was not at his work.’
