- A full waggon of coal being lowered whilst two-thirds filled dirt tub going up, crab in gear and man turning handle, coal waggon stopped, deceased went down the brow to see what was wrong, found the tub off rails, lifted it on rails, remained in the brow pushing down at the full coal tub, the crab came out of gear, the tub of dirt overpowered the coal tub, drew it up brow, deceased being then in front, at a low bar his head was caught and crushed. The pawl on the crab could not have been properly put in position, the jerk of the ropes caused the crab to be put out of gear. Deceased should not have remained in the brow behind the coal tub. Died on 20th October.
