A railway truck was being loaded with slack wheeled from a heap on the pit bank across a gangway over a space of 3ft. composed of two 9in. planks laid side by side. Deceased was in the truck levelling the slack and was ordered out to some other work by the foreman. In stepping across the gangway he slipped or stumbled and fell head first a distance of 8ft. to the ground level.
