- While he was "sledging," a big rock which was resting on some loose stuff at the foot of the slope of a pile of stuff on a stull in a gunniss, a stull piece fell from under his feet, and he, the big rock and other rocks from the pile of stuff fell 15 feet on to the slope of another pile of stuff down which they rolled to a confined space at the top of a shoot, where he was found pinned and partially buried. He died some three hours later before he could be extricated. Apparently the head of the stull piece had been knocked laterally from its hitch by the big rock which had rolled down the upper pile of stuff after being blasted from the footwall on "quitting" at the end of the previous shift. If he had taken the precaution to satisfy himself of the safety of the stull by examining the underside of it from a gangway about 6 feet below, the defect might have been seen and steps then taken to remedy it.
