Deceased was loading a tub on the plates at the top of a jig in the Hardmine seam, when a goth occurred causing a piece of roof about 5 feet long, 2 feet wide and 5 inches thick to fall from over the ends of props set half yard apart. One end of the fall struck him in the back and knocked him against the tub, causing internal injuries. He died December 15th from secondary haemorrhage due to blood poisoning.
