Deceased, with others, was clearing out a sump and filling the material into tubs which were taken up the shaft in the cage. As the cage was approaching a landing in the shaft, a fireman there heard something fall and shouted down, "Look out" Deceased, who had been in safety beneath protecting boards, looked out and was struck upon the head by a piece of brick and killed. The brick had fallen from one of the tubs in the cage.
