He had the controller (tram car type) of a 50-h.p. 3-phase induction motor, for under-ground haulage engine out of its box or case, and was examining it the third time for a defect, when, having omitted to disconnect the current by knocking out the switch at the panel, he touched the terminal and received a 3,000-volt current, although standing on a solid rubber mat. Apparently he must have been touching the box or case at the same time, and thus destroyed his insulation.
