It had been reported to the deceased that the bottom cage was fast in the shaft. He descended another shaft and found the cage was free. He then went on to the top of this cage to examine the shaft. The movement of the cages liberated the top cage sneck, which dropped on to his head, killing him instantly. It was found that it was the top cage that had stuck, and a tub was wedged between it and the shaft side. probably the tub had not been properly snecked.
