It was surmised that Hancock was walking alongside loads in motion, and was attempting to detach a coupling from between them, when they collided with loads at rest and his head was crushed. The practice of coupling and uncoupling either loads or empties while in motion is a fruitless source of accident and managers and others would do well to take strong measures to supress it.
