Deceased was cleansing up at the bottom of the shaft during the breakfast hour. It was the custom during this time for the onsetter to release tire cage by signalling "one" when the winding engineman utilised the time to grease the ropes. This was being done when the cage, coming down very slowly and quietly, crushed Hampson, who was not expecting it, as in general working hours it only descended to the yard mouthing 90 yards above the Arley Seam.
