- Deceased and another shifter each brought a full tub of stone to the top of a self-acting incline coupled by means of chain and cotter, and the brakesman saw the shifter kneeling in the act of coupling the tub received from the bottom and the shifter knocked out the stop block, removed the locker, and then helped push the tubs over the kep on to the incline. The first tub went amain and ran into the two empties at the bottom. The backstay held the empties after they had been knocked back about 2 feet, but the rope transmitted the shock to the remaining full tub making it spring back towards the wheel and throwing it off the rails. Deceased's head was caught between the top of the tub and a girder and he died almost immediately.
