- Deceased was one of a cage load of eight men ascending at the end of the shift when the holding on bar at the top of the cage became loose at one end and he dropped into the cage bottom, and then fell into the shaft and was killed. The bar was fired by the ends passing through the top hoop of the cage, the ends being screwed and nuts attached and the accident was caused by the nuts at one end coming off, due apparently to the threads having stripped. The bar was said to be in order when examined on the morning of the accident, and also when some men descended about midday. Had the cage been fitted with gates, the man's life would have been saved.
