- Deceased was found fatally crushed between the side and a fell tub which he was taking out of a heading. The horse was attached, but no lockers were found in the tub wheels. For a distance of 14 yards from the face of the heading the gradient was against the load, after which the dip outbye for some distance was 1 in 12. Lockers were provided at the brow, but apparently deceased either forgot these as he passed, or attempted to take the tub down without them, with the result stated.
