He was standing behind two horizontal haulage wheels between the full and empty roads on the main rope plan and in a moment of forgetfulness he took hold of the incoming rope and his hand was caught between it and the wheel almost immediately he was caught on the middle of the body between the wheel and outgoing main rope and instantly killed.
