Deceased was engaged at a midworking and when about to put a loaded tub on the cage it moved up and he and the tub were precipitated to the bottom a distance of 44 fathoms. When the cage was in its position at the midworking the engineman neglected to pin down his brake and the weight of the opposite cage with empty tub and rope caused it to descend at the same time causing the cage at midworking to ascend thus leaving the shaft unprotected.
