- Inspection made and inquest attended. Deceased appears to have gone down a distance off our feet to the sump to clear some dirt which he had mentioned about in his report at the beginning of the shift. There was nobody else down in that portion of the mine when he apparently got crushed by the descending cage. He had not signalled that he required the cage at rest although the winding was being carried on from a mouthing above.
