Struck on the head by a lump of coal which fell off a tram as the cage was being placed on the fans. It struck the top of the cage which was at the bottom and glanced off to deceased who was 6ft. from the shaft and was pulling the empty tram away. The penthouse reached to within 9ins. of the cage.
